1. Name of Insurance Company, PPO or HMO.
2. Your date of birth.
3. Insurance ID Number.

• You will be sent a confirmation email when completed, which you will have to click on the link to confirm your appointment.  And a reminder email will be sent 1 day prior to your appointment.

CANCELLATION POLICY FOR CHIROPRACTIC & MASSAGE THERAPY SERVICES 

Please....out of respect for our other patients as well as for our Doctor & therapist, call if there is a change in your schedule.

* There is no charge for appropriately cancelled appointments.  (24 hour notice)

* There is a 50% charge for cancellations under 24 hrs but more than 3 hours.

* You will be billed the full amount for a missed appointment (Less than 3 hour notice or no shows).

* Amazon & Livingsocial Deals will be subject to same cancellation policies.
